Lowering Cancer Rates Among American Indians Requires Better ...

Cancer is often preventable and survivable, but not if you ignore it. ?We have to take cancer on like any other enemy. If you confront it you can beat it,? says Dr. David Perdue, Chickasaw, the medical director of the American Indian Cancer Foundation (AICAF).

AICAF Executive Director Kris Rhodes, a member of the Bad River Band of Lake Superior Chippewa and Fond du Lac Band of Lake Superior Chippewa, will attend the Reservation Economic Summit?s (RES) Celebrating Women in Business Luncheon and Fashion Show on March 1. The luncheon attracts about 2,500 people annually and features a different cause each year; this year?s focus is cancer awareness. Proceeds from a silent auction during the luncheon will benefit the AICAF, and Rhodes will have a booth where AICAF feather ribbon lapel pins will be sold for $5 each, with proceeds supporting cancer prevention and awareness in Indian country. A slide show will feature photographs of Native cancer survivors, those battling cancer and those lost to the disease, such as Elouise Cobell.

According to Rhodes, cancer has become the leading cause of death for American Indians, surpassing heart disease. Lung cancer is the leading cause of those deaths, followed closely by colorectal cancer. ?Disease rates vary by region and tribes, so what is true for the Northern Plains and Alaska may not be true for tribes in the Southwest,? she explains.

American Indians and Alaska Natives have higher rates of lung cancer and colorectal cancer than non-Hispanic white populations (NHW) in Alaska and the Northern Plains. This is attributed to smoking, poor access to health care and cancer screenings, underfunding of the Indian Health Service, a lack of ability to pay for medical treatment and poor awareness of early detection and treatment options.

Rates of stomach, gallbladder, kidney and liver cancers are also higher for American Indians and Alaska Natives than they are for NHWs in Alaska, the Plains regions and in the Southwest. (Reversing that trend, cancer rates overall for American Indians and Alaska Natives were lower than the rates for NHW populations in the Southwest, the Pacific Coast and the East regions, according to a 2008 Centers for Disease Control and Prevention study.)

Perdue cites some other troubling statistics from that report: The number of new cases of colorectal cancer is 39 percent higher in Northern Plains tribes than it is in NHW populations, and Alaska Natives have double the colorectal cancer rate of NHWs. He says one reason those rates are so high could be that colorectal cancer screening rates for American Indians are half that of the NHW population. This is particularly disturbing, he explains, because that kind of cancer is one of the most preventable when patients undergo the proper screening processes and have access to quality care.

The AICAF is working as a subcontractor for the Shakopee Mdewakanton Sioux Community in Shakopee, Minnesota, which got a $625,000 two-year grant funded by the Centers for Disease Control and Prevention through the Indian Health Service to increase colorectal cancer screenings in Northern Plains tribes. Perdue says his team will be visiting 54 tribal health systems in the region to assess each system?s knowledge and procedures for colorectal cancer screenings.

Dr. Judith Kaur, a member of the Choctaw Nation of Oklahoma, is medical director for the Native American Programs of the Mayo Clinic Comprehensive Cancer Center, which provides outreach to American Indian communities. ?We have a tremendously vocal cancer survivor group across Indian country. They are telling family, friends and their communities that if you get screened, diagnosed and treated, there is a good chance you can beat this,? she says. ?In the past, people didn?t talk about cancer because of their fear of it, and it was the general assumption that if you were diagnosed with cancer it was already too late.?

Access to care and screenings are important elements of cancer prevention, detection and treatment, but they aren?t the only weapons in this fight. Mitigating the risk factors is also crucial. The number-one cancer risk factor in Indian country is smoking. The second, according to Perdue, is obesity. ?The biggest thing is: Don?t smoke,? he says. ?Going back to healthier, more traditional diets, cutting out foods high in fats and getting regular exercise will decrease cancer risk.?

Rhodes says cigarette smoke is linked to nine out of 10 cases of lung cancer and one out of three cases of all other cancers. ?We can all contribute to the health of our community by reserving tobacco for sacred purposes only,? she says. ?It is important to ask for help when quitting smoking?ask for help from the Creator, from your family, from your clinic and from your friends.?

Perdue says American Indians often have misconceptions that can be barriers to cancer screening and treatment. Some of his patients believed if they were to talk about cancer they would be more likely to get it. Or that cancer is infectious, or that it comes from breaking taboos. ?People need to understand that cancer is a biologic process and a lot of cancers are treatable and preventable,? he says.

Joy Rivera, Haudenosaunee, is the colon cancer?screening navigator at the AICAF?she is, according to Perdue, their ?face on the street,? the person who talks to tribal members about screenings and walks them through the process. Rhodes says Rivera has been incredibly successful in getting both men and women in following through with their medical providers to request and complete cancer screenings.

Rhodes says there are many barriers to detection and care, such as underfunded health systems, lack of data, lack of programs and professional staff shortages. American Indians in both rural and urban areas have less health-care coverage and less access to health care than the NHW population?added to that are fewer prevention programs, cancer screenings and specialist care targeted to American Indians.

Despite the formidable task she has undertaken, Kaur is hopeful. ?We have more resources today than even five or 10 years ago?but it?s not enough. We need to continually educate new leaders in health promotion and cancer prevention,? she says. ?Now that I am an elder with three grandchildren, I am very concerned that the next generation benefits from both traditional knowledge and western science. There needs to be a lot of dialogue about what new advances there are in cancer treatment.?

What this all boils down to is: don?t smoke, don?t abuse alcohol, don?t eat foods high in fat, do eat lean meats, fruits and vegetables?a more traditional diet. Exercise, even if that just means walking short distances during breaks at work. Do ask your medical providers questions and find cancer screenings appropriate for your risk factors and family history. Don?t ignore, forget, procrastinate or be stubborn about your health.

in the rush to 'e-learning', are we losing sight of ... - Talking Teaching

One of the ?big things? in schools these days seems to be the increasing expansion of e-learning. I?ve written previously on one school?s decision to require all its new students to have iPads, or similar tablet-style computers. At the time I worried about whether, in the rush to embrace new technology, the question of whether its use would enhance student learning was being left behind. ?And a friend of mine who?s a secondary teacher recently said something similar: these technologies can be?tools for learning but do not & should not replace the need for linking our teaching to a student-inquiry-based experiential and cognitive-conflict-based learning (which requires a lot of forethought & planning from teachers!).

That concern resurfaced yesterday as I was reading the NZ?Herald?s on-line edition (on my iPad, lol), & found one story citing a couple of US reports suggesting that perhaps e-learning isn?t all it?s cracked up to be.

The first of the Herald?s references was to this report at?Education News Colorado, which examines the performance of students who are taught entirely on-line (for a range of reasons, that could include having dropped out of ??regular? schooling, living in an extremely isolated area, or for philosophical reasons. At this point I need to note that the news report is based on an analysis of on-line school data, & so far doesn?t appear to have been published in the science education literature. (However, the Colorado Department of Education annual report, from which the data are drawn, can be found here.) Nonetheless, the analysis does appear to highlight some rather worrying trends:

  • Online students are losing ground. Students who transfer to online programs from brick-and-mortar schools posted lower scores on annual state reading exams after entering their virtual classrooms.
  • Academic performance declined after students enrolled in online programs. Students who stayed in online programs long enough to take two years? worth of state reading exams actually saw their test results decline over time.
  • Wide gaps persist. Double-digit gaps in achievement on state exams between online students and their peers in traditional schools persist in nearly every grade and subject ? and they?re widest among more affluent students.

Now, one reason put forward by education officials for the apparently wide differences in results was that on-line education was pretty much an option of last resort, & certainly at least one Colorado virtual school does appear to target at-risk students who may well be behind on many educational indicators. However:

The analysis of state data shows, however, that most online school students do not appear to be at-risk students. Only about 120 students of the more than 10,000 entering online programs last year were identified as previous dropouts returning to school, and only 290 entered online schools after spending the prior year in an alternative school for troubled youth.

The obvious question is, why? Because there does appear to be something going on. And it?s relevant to NZ even though fully on-line teaching is a long way from the use of iPads & their like in a bricks-&-mortar classroom: we?re still looking at two stages on a continuum here.

Part of it could be that kids are not really as tech-savvy as we?d like to think. Putting them in front of a desktop computer, or giving access to things like tablets, doesn?t mean that they?ll necessarily use the technology to its best advantage. They may well need to learn that skill. And those using the technology to teach also need to think about how well it fits their learning objectives ? is it there because it?s ?there?, or because it enhances learning in some way.

Coming back to the full-blown exclusively on-line learning thing: there are also issues of community & pedagogy. In a real (as opposed to virtual) school, students are part of an actual community that includes both their peers & their teachers, & which can extend into the community outside of school. It can be rather isolating to be a distance student, & not be a part of that (this was certainly my experience when I was studying extramurally for my teaching qualification).

Which is where the pedagogy comes in. Certainly from a university perspective, we haven?t always been terribly successful at moving from the face-to-face to the on-line teaching environment. However, technologies like vide0-conferencing, skype, moodle & panopto can help to give some sense of belonging to a learning community ? as can tailoring teaching materials to this alternative means of teaching & learning, instead of simply uploading everything in the format that?s used in ?normal? classes. Are some of the students in the Colorado study missing out on that sense of community?

And the Herald?s?second reference? It was to this story (from September 2011) in the New York Times, which carried out what looks like a fairly extensive investigation on the use of technology in schools, before concluding that

schools are spending billions on technology, even as they cut budgets and lay off teachers, with little proof that this approach is improving basic learning.

Now, that?s talking about the current status quo in parts of the US. New Zealand?s a long way back from what the NYT is describing, both in the extent of our technology roll-out & in the amount of money we have available for it. ?And the research into the effectiveness of on-line teaching & learning is certainly being done (here, here, here & here, for example). (There?s also an interesting review of ?virtual schools? available here, which uses New Zealand as one of its examples.)

But still: technology, in education as elsewhere, is a useful tool, but not necessarily a panacea for all ills.

4 Ways to Foster Attachment to Distant Family Members ...

There are three continents and more than a dozen cities in which we have family. Keeping in touch and helping our children get to know their extended family and form some sort of attachment to grandparents, aunties, uncles and cousins is challenging.

The biggest challenge is somehow creating opportunities for the children to share laughter, milestones and celebrations as well as everyday life so the distance doesn?t feel so great. With face to face visits being limited to a few times a year, in the interim, we have figured out these four ways to help maintain attachment:

Video Chat: My children love to video chat with their distant friends and especially with grandparents and aunties. They also look forward to their daily skype sessions with their papa when he is on business trips. When the children video chat, they do all sorts of silly things like running down the hall singing, playing violin for grampa to listen, show drawings to interested aunties and often running and tumbling over the sofa for a cheering grandparent. With their friends they like to show toys and say lots and lots of funny words, make faces, even hide and re-appear making everyone giggle. All the laughter from both sides I believe really helps the children feel connected even if so far away.

Phone messages: Being almost two, Bella is fascinated with the telephone. She has connected the idea that if. someone is out of sight they might be reachable on the phone. One time when handsome hubby was travelling he left a voice mail and Bella was so fascinated with it she now loves to listen to phone messages and talk back. Sometimes when she wants to talk on the phone with time zones and work schedules, a live conversation doesn?t work but the recording gets her the connection to that voice she wanted.

Videos: We send monthly or bi-monthly video compilations to grandparents and great-grandparents as a way for them to be a part of the childrens development, see their latest play inventions. There are times when the boys want to create deliberate messages within the videos, otherwise we just send snippets of our daily life.

Mementos: When we do have a chance to have face to face visitors, they often bring gifts. These become very special to the children. For example, my mother brought some hats from a trip to china and the children love wearing them for pretend play and often tell me they are special from when VoVo (grandmother in Portuguese) visited us. Another special item is my fathers winter hat which three years ago my oldest became very attached to during a visit and asked to keep. He still cares for the hat, wears it to play in the snow but always reminds us that it belongs to his grandfather. Other special items are a book written and illustrated by one of my sons best friends from the states and a picture of his buddy at a firehouse visit. My three year old likes to look at a binder with all his projects from his former preschool in Germany.

Hello,

I am a Peace Corps Youth Development Volunteer currently serving in Morocco. I am working at a Sports and Youth Center teaching English. I have been in my community for almost three months now and I am still in the process of developing the English Language program at my center. The majority of my students are boys between the ages of 8 and 10 and I have been having a little trouble getting them to focus and participate in class. I think I have been having such difficulty because I haven't been able to find a topic that excites them and that they are comfortable talking about. That was until I discovered how much they all love WWE. Most of my students are too shy or embarrassed to talk to me in English even though I know they can, unless I ask them about the WWE and they can tell me all about John Cena, Undertaker and all the other WWE Superstars. ?

So I decided to plan a lesson where each student drew a picture of their favorite WWE Superstar and then told the class about them. They really enjoyed the activity and I was amazed at how engaged and excited they were to use their English to tell me all about their favorite WWE Superstar. There was one student who I have never heard speak English, and who I assumed came to class because his older brother did, but he was able to tell me all about his favorite WWE Superstar (John Cena) and I was truly amazed! ?

As with speaking any new language, half the battle of learning it is having the confidence to speak it, and this lesson was a great opportunity for my students to do just that. WWE Teachers' Corner was the inspiration for my lesson and I plan to incorporate the WWE in more of my lessons to help build speaking confidence. ?

Motorists hit by record surge in gas prices

Gasoline prices have never been higher this time of the year in the U.S.

At $3.53 a gallon, prices are already up 25 cents since Jan. 1. And experts say they could reach a record $4.25 a gallon by late April.

"You're going to see a lot more staycations this year," says Michael Lynch, president of Strategic Energy & Economic Research, referring to people staying at home on their vacations. "When the price gets anywhere near $4, you really see people react."

Already, W. Howard Coudle, a retired machinist from Crestwood, Missouri, has seen his monthly gasoline bill rise to $80 from about $60 in December. The closest service station is selling regular for $3.39 per gallon, the highest he's ever seen.

"I guess we're going to have to drive less, consolidate all our errands into one trip," Coudle says. "It's just oppressive."

The surge in gas prices follows an increase in the price of oil.

Oil price surge could dampen recovery

Oil around the world is priced differently. Brent crude from the North Sea is a proxy for the foreign oil that's imported by U.S. refineries and turned into gasoline and other fuels. Its price has risen 11 percent so far this year, to around $119 a barrel, because of tensions with Iran, a cold snap in Europe and rising demand from developing nations. West Texas Intermediate, used to price oil produced in the U.S., is up 4 percent to around $103 a barrel. That's 19 percent higher than a year earlier.

Higher gas prices could hurt consumer spending and curtail the recent improvement in the U.S. economy.

A 25-cent jump in gasoline prices, if sustained over a year, would cost the economy about $35 billion. That's only 0.2 percent of the total U.S. economy, but economists say it's a meaningful amount, especially at a time when growth is only so-so. The economy grew 2.8 percent in the fourth quarter, a rate considered modest following a recession.

High oil and gas prices now set the stage for even sharper increases at the pump because gas typically rises in March and April.

Every spring, refiners suspend operations to switch the type of gasoline they make. Supplies of wintertime gas are sold off before March, when refineries need to start making a new formula of gasoline that's required in the summer.

That can mean less supply for service stations, resulting in higher gas prices. And summertime gasoline is more expensive to make. The government mandates that it contain less butane and other cheap organic compounds because they contribute to the formation of ground-level ozone, a primary constituent in smog. That means more oil, a costlier component, is needed to produce each gallon.

The Oil Price Information Service predicts that gasoline could peak at $4.25 a gallon by the end of April. That would top the record of $4.11 in July 2008.

The national average for gasoline began the year at $3.28 a gallon. The average price for February so far is $3.49 a gallon. That's up from $3.17 a gallon last February, a record at the time. Back in 2007, before the recession hit, the average for February was $2.25 a gallon.

Prices are higher on the East and West Coasts, where gasoline has risen above $3.70 in Connecticut, New York, Washington D.C. and California. This isn't unusual ? states on the coasts charge some of the nation's highest gas taxes.

High gas prices put a strain on many people's budgets.

Americans spent 8.4 percent of their household income on gasoline last year when gas averaged an all-time high of $3.51 a gallon. That's double the percentage a decade ago. They could pay even more this year, even though demand is the lowest in 11 years as people drive fewer miles in more efficient cars, says Tom Kloza, chief oil analyst at OPIS.

Goodman, an accountant, commutes by bus. He uses his car mostly for trips to the grocery store or for occasional nights out. He says he has no choice but to eat the higher gas costs.

Paul Dales, a senior economist at Capital Economics says it would take a bigger shift in the global economy ? say, a deep recession in Europe or a slowdown in Asia's manufacturing ? for pump prices to drop noticeably. Either event would slow oil demand, depressing prices.

But experts expect demand to keep rising. World oil demand is expected to increase by another 1.5 percent to 89.25 million barrels a day in 2012, according to the Energy Information Administration.

In the short term, tensions with Iran are feeding fears that oil supplies could be blocked.

The U.S. and Europe are tightening economic sanctions against Iran over what the West believes is Iran's attempt to build a nuclear bomb. World leaders fear Israel may be planning a strike against Iran, the world's third largest oil exporter.

In response, Iran has threatened to withhold its own oil deliveries and to block the Strait of Hormuz, a waterway along its coastline through which one-fifth of the world's oil flows.

On Friday, an international banking clearinghouse crucial to Iran's oil sales said it is prepared to discontinue services to Iranian financial institutions being targeted by the EU and U.S. sanctions. That could ratchet up the pressure on Iran, but also send oil prices soaring.

The price of Brent crude fell 53 cents on Friday to $119.58. WTI gained 93 cents to $103.24.

Gas prices are already an issue in the presidential campaign. Republican candidate Newt Gingrich spoke several times this week about opening up more federal land to oil and gas drilling as a path toward U.S. energy independence ? and lower pump prices.

"Our goals should be to get gasoline to $2.50 or less so that working families can actually get to work and retired families can travel," Gingrich said at a campaign event in Los Angeles Thursday.

Manufacturers providing outsize boost to economy (AP)

WASHINGTON ? American factories are humming ? and driving the economy forward.

Manufacturers have been hiring more consistently than other employers, for jobs with better-than-average pay. They just had their best month of growth in five years. And more factory output has raised demand in some other industries, such as shipping, leading to further hiring.

"The manufacturing sector is on a tear," said Paul Ashworth, an economist at Capital Economics.

It's an optimistic theme that serves President Barack Obama's political needs. On Wednesday, Obama traveled to Milwaukee to salute a company that brought jobs back to the United States. The president has promoted the nation's manufacturing base as an engine of growth and as evidence of a recovering economy.

No one thinks manufacturing will return to its 1950s peak. After all, the factory sector now makes up barely one-tenth of the economy.

But since the recession ended more than 2 1/2 years ago, factories have been contributing disproportionately to the recovery in hiring and the overall economy.

A big reason, economists say, is that individuals and businesses are making major purchases they delayed during the Great Recession and its aftermath. Consumers are buying more cars and appliances. Companies are investing in industrial machinery and computers.

The release of that pent-up demand gives manufacturing a kick that isn't visible in some other corners of the economy. Manufacturing was hit particularly hard by the recession. Consumers postponed purchases of cars, refrigerators and flat-screen TVs, even as they continued to visit doctors, get haircuts and pay utility bills.

"Manufacturing has punched above its weight, but that's because it was punched in the stomach in the recession," Michael Montgomery, a senior economist at IHS Global Insight, said.

Factory output got off to a robust start this year, and it ended last year with the fastest growth in five years, the Federal Reserve said Wednesday.

Those were the best back-to-back monthly performances since summer 2009, when the recession ended, according to Jonathan Basile, an economist at Credit Suisse.

Manufacturing is delivering an outsize benefit to the economy in key ways:

? JOBS:

About 9 percent of the nation's jobs are in manufacturing. But last year, factories added 13 percent of new jobs. And in January, about one-fifth of the 243,000 net jobs the economy created were in manufacturing.

Factory growth has also helped to increase hiring in other industries such as shipping, warehousing, department store sales and auto sales. Railroad operators such as Union Pacific have stepped up hiring as their shipments of cars, machinery and other equipment have climbed.

The hiring has boosted struggling Midwestern states such as Ohio and Michigan, which will likely be battleground states in the presidential election.

George Mokrzan, an economist at Columbus, Ohio-based Huntington Bank, said those two states have added a greater percentage of jobs since the recession ended than the nation as a whole.

? WAGES:

Average hourly pay for factory workers making durable goods, such as autos, was $20.15 in January, according to the Labor Department. That's above the average of $19.37 for the broad service sector, meaning that each new manufacturing job, on average, fuels more consumer spending than does the equivalent new service-sector jobs.

The service sector, which employs roughly 90 percent of the private-sector workforce, includes everything from restaurants and hotels and retailers to financial service firms and construction companies.

? CONFIDENCE:

As Clint Eastwood's Super Bowl halftime ad for Chrysler ("Our second half is about to begin.") showed, Americans are thought to respond more emotionally to a rebound in manufacturing than in other sectors. The perception that American manufacturing is healthy again could boost confidence in the economy, some analysts say.

Jeffrey Bergstrand, a finance professor at Notre Dame University, calls it "factory nostalgia" but says that it is "economically legitimate."

"It reminds us of the period of the greatest growth we had in the U.S. economy," Bergstrand said. "It's also ... an association with the boom in the middle class, (when) they were all sharing in that economic boom."

Still, economists caution that expanding factories can do only so much to reduce unemployment. That's because factories have increasingly relied on automation and other advances to produce more goods with fewer people.

Many economists expect manufacturing to keep growing faster than the broader economy for the rest of the year. It's not clear how long that will continue.

"This is nothing more than the manufacturing sector recovering faster because it got hammered worse," Montgomery said.
